Pain Management Plan & Clinical Assessment

Our program focuses on providing a community-based solution for clients living with persistent pain. This treatment approach works with local clinical and educational experts while ensuring our outcomes complement existing institutional (hospital-based) options.

The development of your pain management plan involves a multidisciplinary clinical assessment coordinated by Pain Education and Management in partnership with your GP and local allied health professionals.

 

The process begins with your medical practice, where your GP provides a comprehensive medical history. This, along with your responses to the pain questionnaire, is shared with the assessing clinical team.

 

Pain Education and Management then arranges individual appointments with our clinicians (psychologist, physiotherapist, and pharmacist). Following these assessments, we collate the findings and provide a summary report to your practice.

 

A case conference is then scheduled, bringing together your GP and our clinicians to review the results and develop your tailored pain management plan.

 

Finally, you will meet with your GP to discuss your plan and agree on the best way forward in managing your pain.

 

This package includes an allocation for allied health assessments and administration charges but excludes GP visit costs. It also assumes that patients will be managed on a chronic condition management plan by their GP. 

 

Medicare rebates or DVA funding may be available based on individual entitlements. We suggest checking with your GP and allied health providers for confirmation of actual costs.
