Work With Us

A Shared-Care Multidisciplinary Pain Management Pathway
Pathways to Pain Management is a structured, evidence-informed pain education and support pathway designed for use alongside primary care and allied health treatment.
We work with GPs and allied health clinicians through a shared-care model, providing education, coordination, and a multidisciplinary structure that supports, not replaces, your clinical care.
Your client remains your client. We add the program framework around your care.

A Plug-In MDT Pathway — Not a Client Transfer Program
Our program provides:
-
Structured pain education
-
Client preparation and activation
-
Multidisciplinary assessment pathways
-
Care coordination and navigation support
-
GP communication and case conferencing
-
Outcome tracking and reporting
Partner clinicians provide their discipline-specific care within this coordinated pathway.
Think of the program as an MDT overlay and coordination spine — not a replacement service.
Program Recognition

Formally endorsed by Painaustralia, Australia’s national peak body for pain advocacy and policy.
Recipient — 2024 Pain Sector Award for Best Multidisciplinary Care Program, recognising excellence in coordinated pain care.
For Allied Health Clinicians

Join the Provider Network
Allied health partners can both:
✅ Refer their own clients into the program
✅ Receive appropriate program clients needing their discipline
When You Refer a Client
-
Your client remains under your care for your discipline
-
Program education reinforces your clinical messaging
-
We support engagement between visits
-
MDT input can be added where appropriate
-
Care plans are shared and coordinated
When We Refer to You
-
Program participants may be matched to you based on:
-
Discipline
-
Location
-
Availability
-
Existing treating relationship
-
Client preference
-
We operate a shared-care, referral-reciprocal model. We don’t take clients away — we build structured support around your care.
For General Practitioners

GP Referral Pathway
GPs can refer patients into the program at any time.
Formal registration is optional. Most practices refer when clinically appropriate — no subscription or ongoing commitment is required.
Registered GP practices can optionally receive:
-
Referral templates
-
Practice resources
-
Program updates
-
Case conference coordination support
-
Patient progress summaries
The program is designed to complement usual GP care and reduce the burden of persistent pain management in short consultations.
Education Partners (Invitation Based)
