One Program – Multiple Referral Pathways

At Pain Education and Management, we’ve developed a flexible, evidence-based 4-step chronic pain management program designed to fit seamlessly into your practice and enhance the care you provide to patients living with chronic pain. Built on clinical expertise, research, and lived experience, the program is both practical and patient-centred.

Our program offers a comprehensive, flexible approach to chronic pain management. Patients begin with free education, then progress to more personalised care as needed, guided by their clinician’s recommendations. This empowers patients to take control of their pain management journey, with access to the right support, education, and resources at every stage.

Step 1 – Self-Directed Education
Free for patients referred by a partnering clinician.
Patients access our 11-module online program, designed in plain language and self-paced, to learn proven strategies for managing chronic pain at home.

 

Step 2 – Clinician-Led Consultation
For patients needing more personalised support.A one-on-one consultation via telehealth or in-person as part of a group workshop to expand on program content, review patient needs, and goals, with options for providing tailored guidance and recommendations.

 

Step 3 – Clinical Assessment
For personalised care and treatment planning.
Comprehensive musculoskeletal, psychosocial, and pharmacological assessments. Recommendations are documented in a personalised pain management plan, which can be reviewed in a GP case conference.

 

Step 4 – Ongoing Support & Monitoring
For long-term patient empowerment.
Continued education, clinician support, and health coaching, with access to preferred rates from our allied health provider network to support active self-management.

Patients can choose from any of the available services; however, Step 1: Self-Directed Education is designed as a free entry point for those referred by a partnering clinician. This step saves patients $105, the cost of self-referral, and aligns with our preference for clinician-supported engagement throughout the program. Patients can then escalate their care based on their needs and clinician recommendations.

We provide four flexible referral pathways:

  1. Patient Self-Referral

    • Case Review – Comprehensive consultation (non-clinical) reviewing the patient’s situation, exploring multidisciplinary team options, and identifying Medicare funding gaps.
      Cost: $150 (includes full Step 1 access)

    • Program Enrolment Only (Step 1) – Direct access to our complete 11-module online program.
      Cost: $105

  2. GP Referral

    • Referred as part of a GP Chronic Condition Management Plan (GPCCMP), with Step 1 included for education and self-management.
      Cost: Patient – Free | Practice – Free

  3. Education Partner Referral

    • Educators refer patients to augment existing support.
      Cost: Patient – Free (Step 1) | Practice – Free

  4. Allied Health Referral

    • Allied health professionals refer patients to support and complement treatment.
      Cost: Patient – Free (Step 1) | Practice – $10/month subscription
