Start the Pathways to Pain Management Program

The Pathways to Pain Management Program is provided free of charge with a referral from your GP, saving you $105.

To access this free option, follow these simple steps to download your referral form and get started.

1. Download this form

​​2. Take It to Your GP

Bring the printed form to your GP appointment.
Ask your GP to:

  • Fill in their details

  • Tick the referral option

  • Sign and date the form

  • Hand the form back to you

3. Upload the Signed Form

Once your GP has signed the form:

  1. Scan the QR code on the form, or

  2. Click the button below ​

4. Begin Your Program

After submission, you’ll be automatically enrolled.

  • You’ll be redirected to Module 1 to create your login (email + password)

  • The same login works on both the website and the app

  • You’ll also receive an email with a direct link to Module 1, in case you aren’t redirected automatically.​

  • Didn’t get redirected? → Check your email for the Module 1 link

  • Forgot your password? → Use Forgot Password on login
