Start the Pathways to Pain Management Program

The Pathways to Pain Management Program is available free of charge with a referral from your Specialist, GP or allied health provider, saving you $105.

To access this free option, follow the simple steps below to download your referral form and get started.

If you already have a referral from your Specialist or GP, or have been referred by an allied health provider, please proceed directly to Step 3.

1. Download this form

​​2. Take It to Your GP

Bring the printed form to your GP appointment.
Ask your GP to:

  • Fill in their details

  • Tick the referral option

  • Sign and date the form

  • Hand the form back to you

​

3. Complete your Registration

Once you have a completed referral:

  1. Scan the QR code on the form, or

  2. Click the button below ​

4. Begin Your Program

After submission, you’ll be automatically enrolled.

  • You’ll be redirected to Module 1 to create your login (email + password)

  • The same login works on both the website and the app

  • You’ll also receive an email with a direct link to Module 1, in case you aren’t redirected automatically.​
