Pain Management Plan

Our program focuses on providing a community-based solution for clients living with persistent pain. This treatment approach works with local clinical and educational experts while ensuring our outcomes complement existing institutional (hospital-based) options.
The process of developing your pain management plan involves a multidisciplinary clinical assessment organised by Pain Education and Management working with your GP and local Allied Health professionals.
It starts with your medical practice, where a comprehensive history and neurological examination are completed. This information, various referrals and your pain questionnaire responses are provided to the rest of the assessing team.
Pain Education and Management will arrange appointments with our clinical team (psychologist, physiotherapist and pharmacist). We will collate the results of these assessments and report them to the practice before scheduling a case conference. The case conference allows the GP to review the overall assessment with our clinicians; from this, we will draft your pain management plan.
Finally, you will meet with your GP to discuss your plan and agree on the best way forward in managing your pain.
